If you have been involved in a traffic accident or become the victim of an incident.  You can request a copy of your report by filling out the Report Request Form  In order to locate the incident we will need you to complete the form as thoroughly as possible.  Case or CAD number of the incident, date and time of the incident, where the incident occurred.  Your name and your contact number.
