Palomar Health Rotation Requirements

When naming your documents to upload, please use file names that describe what is on the document. Do not use file names like Image_001.

If any of the following requirements will expire during your rotation, it is your responsibility to renew this paperwork during your rotation and contact the staff member in charge of approving Complio requirements so they can share the updated document with the site. Failure to do so will result in your approval with the site being revoked until it is reapproved.

Required Vaccinations

  • COVID-19 + Booster (must submit one of the following)
    • A) One dose of the bivalent COVID-19 vaccine (Pfizer or Moderna only) – documentation must clearly indicate that it was a bivalent vaccine
    • B) Documentation for your initial COVID-19 vaccine series (1 dose J&J, 2 doses of either Pfizer, Moderna, or Novavax) AND 1 dose of a COVID-19 booster completed 2 months after the last dose of your initial series (only Pfizer, Moderna, or J&J are eligible to be boosters)
    • C) Palomar Health’s COVID-19 Vaccine Declination Form (located in the Student Health Clearance Packet)
  • Flu Vaccine (seasonal/annually done)
  • Hepatitis B
  • TDap (good for 10 years)
  • MMR
    • Measles Titer + Mumps Titer + Rubella Titer
    • MMR (2-Dose)
  • Varicella (2-Dose or Titer)

Other Documents

  • Background Check* (good for 7 years)
  • CPR Certification (good for 2 years)
  • Drug Screen* (good for 7 years)
  • Tuberculosis (needs to be done annually)
    If this is your first placement or if 180 or more days have elapsed since your last placement, your most recent TB test must have been completed within 90 days before the start date of your placement. If you are a returning or current student, you only need to update your TB before it expires.

*Background Checks and Drug Screenings do not seem to be pulling over from your school Complio account to this rotation. Please make sure these documents are uploaded for these requirements for clearing.
