Use the following form to either request copies of past student course evaluations or to request that student course evaluations be conducted outside of your normal evaluation cycle.
For Distinguished Faculty Award
If you have been nominated for DFA, you are required to provide one class of student evaluations conducted within the last 3 years. You can use this form to request a copy of your student evaluations, or if you do not have one class of student evals within the last 3 years, you can request that student evaluations be conducted specifically for the DFA committee.
If you are requesting new evals specifically for DFA they will run for one week only, so it’ll behoove you to remind your students to complete the evaluation promptly. Evaluation results will be sent directly to the DFA committee, you will not be able to view the results until after grades have posted.

Use this form to either request to have student evaluations conducted in your course outside of your regular evaluation cycle or to request past copies of student course evaluations that were already completed.
