Part-Time Evaluation Report Submission

Use this form to submit the required Part-Time Evaluation documentation.

TERB will route the evaluation report for signatures. Meet with your evaluee after the evaluator and department chair have signed the evaluation report. Meeting with the evaluee to review the final evaluation report is a required and important part of the evaluation process.

Required PT Evaluation Documents

  • A completed Department Chair form (this is located at the end of the evaluation report, you can separate this or keep it within the report, the content from the form is used in writing the evaluation report)
  • A completed observation form
  • A completed evaluation report (double check that all ratings are noted and objective comments are included. If the rating comments include direct student eval quotes then you cannot share the report with the evaluee until after grades are posted. If the comments including paraphrased or summarized student comments then you can meet with your evaluee before the semester ends.)
  • Optional completed Dean Form (Check with your department chair to determine if the Dean form is required)
  • If required, Draft Improvement Plan
    • If the PT Faculty has a PN and receives a “Performance needs improvement” or “Unsatisfactory performance”, then you are required to also submit a DRAFT Improvement Plan (IP) along with the final evaluation report. After the evaluee has reviewed the final evaluation report, the DRAFT IP should be used to develop the final IP in a collaborative way between Evaluator(s) and the Evaluee with consultation from the TERB Coordinator. 

Evaluation Documents Structure

For all documents, PDF’s are appreciated.

Please use the following naming practices for the documents:

  • Evaluee last name, first name_document name_semesteryear
    • example:
      • “Falcone, Kelly_PT Eval Report_F22”
      • “Falcone, Kelly_Observation by CRose_SPR22”
      • “Falcone, Kelly_ Chair Form_ F22”
    • Please do not include your name, as the evaluator, in the file name, it is unnecessary and is removed from the filename in the TERB files.
