Probationary Spring Meeting Confirmation Upload (PDF Only)

The Spring Meeting Confirmation form is due by the last day of the spring semester.

Use this form to submit the required Probationary Evaluation documentation- both Fall reports and Spring Confirmation form.

For the Fall evaluation, this includes the final evaluation report, observation forms, chair letter, self-evaluation, and PD transcript. If any discrete item in the report received a Needs Improvement or Unsatisfactory, an Improvement Plan is required as well.

For the Spring evaluation you are only required to submit the Spring confirmation form.

Before submission, please ensure all documents are complete. Please double check that every item on the evaluation report has a rating and includes comments that support/explain the rating. If the rating comments include direct student eval quotes then you cannot share the report with the evaluee until after grades are posted. If the comments including paraphrased or summarized student comments then you can meet with your evaluee before the semester ends.

Meeting with the evaluee to review the final evaluation report is a required and important part of the evaluation process.

Please use the following naming practices for the documents:

  • Evaluee first initial last name_document name_semesteryear
    • example: “kfalcone_Evalreport_spr22” or “kfalcone_observation_F22”
