Probationary Evaluation Report Submission

Use this form to submit the required Probationary Evaluation documentation- both Fall and Spring.

TERB will route the report for signature. After the committee members, Dean, and VP have signed, then meet with the evaluee to review the report and materials. Meeting with the evaluee to review the final evaluation report is a required and important part of the evaluation process.

Fall Semester: Required Probationary Evaluation Documents

  • 3-4 completed observation forms (3 required, 4 possible)
  • Evaluees PD Transcript
  • Evaluees Self Evaluation
  • Department Chair Letter
  • completed evaluation report (double check that all ratings are noted and objective comments are included. If the rating comments include direct student eval quotes then you cannot share the report with the evaluee until after grades are posted. If the comments include paraphrased/summarized student comments then you can meet with your evaluee before the semester ends.)
  • If required, Draft Improvement Plan
    • If just one discrete item for Probationary is “Performance needs improvement” or “Unsatisfactory performance”, then you are required to submit a DRAFT Improvement Plan (IP) along with the final evaluation report. After the evaluee has reviewed the final evaluation report, the DRAFT IP should be used to develop the final IP in a collaborative way between the TEC and the Evaluee with consultation from the TERB Coordinator. The Probationary IP’s are also reviewed/approved by the TERB Committee. Please contact the TERB Coordinator if an IP is required.
  • The final evaluation report and supporting materials are due to TERB by the last day of the fall semester.

Spring Semester: Required Probationary Evaluation Documents

  • Spring Meeting Confirmation Form
  • The form due to TERB by the last day of the spring semester.

Evaluation Documents Structure

For all documents, PDF’s are appreciated.

Please use the following naming practices for the documents:

  • Evaluee last name, first name_document name_semesteryear
    • example:
      • “Falcone, Kelly_Prob Eval Report, yr1_F22″
      • “Falcone, Kelly_observation by CRose_F22”
      • “Falcone, Kelly_PD Transcript_SPR22”
      • “Falcone, Kelly_Self Eval_SPR22”
      • “Falcone, Kelly_Chair Letter_F22”
    • Please do not include your name, as the evaluator, in the file name, it is unnecessary and is removed from the filename in the TERB files.
