Reminder: All changes must be submitted via digital form. Adding a dependent? Be sure to turn in the following documents: 1) adding spouse: marriage certificate & 1st page of most recent tax return, 2) adding dependent children: birth certificate. Documents can be emailed to: Benefits Office

The Benefits Office has scheduled walk-in events to answer questions and provide support (held in LRC-308)
SISC will host a virtual presentation and Q & A during our August 26th drop-in date.
Our EAP provider will host 3 presentations, 2 on-site with a virtual option and 1 only virtually.
Don't forget, annual enrollment is required for all flexible spending accounts. American Fidelity will assist with flexible spending account enrollment and other American Fidelity products on-site and virtually.
AFLAC will be on-site to assist with questions and enrollment.

What you need to do to make a change on or before August 29, 2025
1.Find the plan that you would like to change from the list below
2.Click on that plan text to pull up the digital form
3.Carefully complete the fields on the digital form
4.Use the “sign” button to create a signature and sign the form
5.You will receive an emailed copy of the completed form as confirmation of your submission
6.The Benefits Office will automatically receive a copy of the form
