Proof of immunizations for pertussis, measles, TB, and flu (optional) are required by California law.

Volunteers must show and obtain proof of immunizations before their first lab or service learning start date. 

This Immunization Record Form has been created with our students in mind. Please make an appointment with our Student Health Centers on campus and your current immunization record to the appointment. 

Upon completion, please turn this form to your Child Development/Education Professor.
