Please fill out this form when you would like a student to make up a test in the TLC.  For full proctoring services, please call the STEM Center on the main campus (760) 744-1150, ext. 2729.

Instructor Procedures:

1. Fill out the provided PDF:    TLC-Make-Up Test Form             For DRC Students: DRC Exam Form
2. Provide a physical copy of the PDF and the original test to the TLC and give it to Nick or Andrea.
3. Pick up the test, have it put in your mailbox, or scanned back if you will be off campus for an extended amount of time.
