You can use this form to request a copy of your student evaluations, or if you do not have one class of student evals within the last 3 years, you can request that student evaluations be conducted specifically for the DFA committee.<\/p>\n\n\n\n<p>If you are requesting new evals specifically for DFA they will run for one week only, so it&#8217;ll behoove you to remind your students to complete the evaluation promptly. Evaluation results will be sent directly to the DFA committee, you will not be able to view the results until after grades have posted.  <\/p>\n\n\n<script>\nvar
