Log into the <a href=\"https:\/\/myhealth.palomar.edu\" target=\"_blank\" rel=\"noreferrer noopener\">Patient Portal<\/a><\/p>\n\n\n\n<p>2. Click on \u201cSend a Secure Message\u201d<\/p>\n\n\n\n<p>3. Select \u201cNew Message\u201d<\/p>\n\n\n\n<p>4. Select the type of message you would like to send and click &#8220;Continue&#8221;<\/p>\n\n\n\n<p>5. Select&nbsp; &#8220;Add attachment&#8221; and choose file or picture (if submitting by phone) to upload your form<\/p>\n\n\n\n<p>6.
