Log into the <a href=\"https:\/\/myhealth.palomar.edu\" target=\"_blank\" rel=\"noreferrer noopener\">Patient Portal<\/a><\/p>\n\n\n\n<p>2. Click on \u201cSend a Secure Message\u201d<\/p>\n\n\n\n<p>3. Select \u201cNew Message\u201d<\/p>\n\n\n\n<p>4. Select the type of message you would like to send and click &#8220;Continue&#8221;<\/p>\n\n\n\n<p>5. Select&nbsp; &#8220;Add attachment&#8221; and choose file or picture (if submitting by phone) to upload your form<\/p>\n\n\n\n<p>6. Make sure to check the box where it states \u201cI confirm I have attached the completed form\u201d before you click &#8220;Send&#8221;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Accessibility Statement<\/h2>\n\n\n\n<p>We are committed to making all digital content for the Student Health Center accessible to every student.
