The following documents form the application package:

2. Income Verification (ONE of the following)

a. Signed copy of your federal income tax form 1040 if you are an independent &/or a Veteran Affairs recipient under the age of 24.

b. Signed copy of your parents/spouse's (or other person who claims you) federal tax form 1040 if you are a dependent and/or under the age of 24.

c. If student/parents did not file income taxes, submit an Income Verification Form [ dependent - Independent ] (also available in TRIO-SSS office) completed and signed along with supporting documentation from EDD/Social Security/Social Services for unemployment, off/on disability, AFDC, etc.

3. Copy of Social Security Card

4. Proof of Citizenship/Legal Residence

a. If Checked “permanent Resident” -resident/alien card

b. If checked “Other”- supporting documentation

5. Proof of Disability

a. If checked “I am a DSPS student,” either a completed DRC verification form or verification from a doctor or appropriate individual.
