Print Services Forms

We have three forms that must be completely filled out before work can proceed.

Creative Services - to start the creative pre-press and press production of the job. Please print three copies and send two completed and signed forms to us and keep one for your department.
Comet Copy - for any copy job. Please print two copies and send one completed form to us and keep one for your department. If you have a full version of Acrobat, you may also save the .pdf after you fill it out.
Comet Copy form 2-up - Use this form if you would like to print out your work orders.
Paper order - Please print two copies and send one completed form to us and keep one for your department. If you have a full version of Acrobat, you may also save the .pdf after you fill it out.

These forms must be completely filled out before work can proceed. They can be filled out on your computer and printed with the appropriate number of copies. If you have a full version of Acrobat, you may also save the .pdf after you fill it out.

Complete the "Work Order" at least 3 to 4 weeks before your project is due. The work order must have:

a signature of a faculty or staff member,
the actual due date. If you write “ASAP” your work order will be returned. If you are not sure of a date, please call us and we will let you know the earliest it can be completed,
a copy code. If you do not have a copy code for the budget string to be billed, please contact Anna Morrison to create one for you.

If any of the above information is missing, your project can not be started. Bring the work order to Print Services along with all information concerning your project, such as layout ideas, all necessary artwork, photography and logos.
