Social Security Field

This field contains your social security number. It need not be supplied if you fill out the next field, an employee ID.

 If you have never submitted a contract to the Professional Development office, you need to submit a valid SSN.

The format of this field is 9 digits, optionally with dashes. For example: 123456789 or 123-45-6789.
