Printing Directions:

  1. Double click on the form you are interested in.

    • (The form will appear in a different window)

  2. Please do not attach any of these forms to an email since original signatures are required for processing.

  3. Click on the printer icon located on the Tool Bar.

    • (The form will be routed to your default printer.)

Note: Classified Absence Reports are not available in the payroll
documents. If you are classified, and lost your absence report, please contact Cyndi Battaglia.
