DATA AND FORMS
For
Noncredit Tutoring
Follow the instructions below to access and save
the
Institutional Review and Planning form for Non Credit Tutoring.
Instructions for Downloading Forms for the
Tutoring Division
1. Click on
TUTORING Disc TUTORING Non-Credit.
2. Click on the SAVE
button to save the file to your
computer. Please do not open the document and enter information into the
online version of the form.
3. Once you have saved the file to your computer,
open it through Microsoft Word and complete your form.
SUMMARY DATA: If you are interested in
summary data for the college, click
COLLEGE.
SUBMISSION INSTRUCTIONS:
1. Send
your completed hard copy of the form no later than 3/14/08
2/14/08 by
campus mail to the Office of Instructional Services for review by
IPC.
2. Also
send an electronic copy no later than 3/14/08
2/14/08 by email
to
jdecker@palomar.edu
