INSTITUTIONAL REVIEW DATA AND FORMS

On this page, you can access Institutional Review and Planning data and forms for your department and disciplines. The Instructional Planning Council is asking that departments complete a Review and Planning form for each discipline within their department.

Follow the instructions below to access and save your Institutional Review and Planning forms.

Instructions for Downloading Forms

1. Click on a discipline within your department (e.g., AMINDIAN Disc AIS).

2. Click on the SAVE button to save the file to your computer. Please do not open the document and enter information into the online version of the form.

3. Complete this activity for all disciplines within your department.

4. Once you have saved the files to your computer, open them through Microsoft Word and complete your forms.

5. Department summary data, which represents all disciplines summed together is available by clicking on the department link (e.g., AMINDIAN Dept)

NOTE: Single discipline departments will have a department form only. Just click on the department link (e.g., CHILDDEV Dept) to get your form.

SUMMARY DATA: If you are interested in summary data for your department, you can select your department file and perform the same procedures. If you are interested in summary data for your division, click the first Division file. If you are interested in summary data for the college, click COLLEGE.
