PRP Instructions, Data & Forms for Academic Year 2012-2013

The links below provide access to the
PRP forms. Every academic discipline, department, and instructional support unit
must complete a PRP. The Instructional Planning
Council is asking you to
(1) Click here to review directions for completing the PRPs
(2) Provide a hard copy to the Division Dean no later than September 14, 2012
(3) Provide a hard copy with the Dean's sign-off to
Instructional
Services by September 28, 2012
(4) Email an electronic
copy to
jdecker@palomar.edu by September 28, 2012
Instructions for Downloading Forms
A. Click on the applicable discipline or department-level link. (Note: The department-level form is only for departmental requests for resources that "will be shared by two or more of the department's disciplines." All disciplines must complete Steps I and II of their discipline form at a minimum even if resources are not being requested.)
B. Click SAVE button to save each file to your computer. Please do not open the document and enter information into the online version of the form.
C. Open the saved file in Microsoft Word and complete the form. Do not add additional form fields! All items you want considered must be typed in the appropriate box.
(NOTE: Do Not
Attach Documents to the form. All information must be
entered into the PRP form.) At the end of the form, list the faculty and staff who participate
in completing this PRP. Save the completed form and
(1) Provide a hard copy to the Division Dean no later than September 14, 2012
(2) Provide a hard copy with the Dean's sign-off to
