Please note, as stated on the Service Provider contracts, all approving signatures must be obtained before the services begin.

Service Provider Contracts:
Agreement for Service Education (Academic)
-Full time or part time faculty, or academic administrators providing educational services beyond their normal scope of work.

Agreement for Service Provider (Classified)
-Current Classified employees providing services beyond their normal scope of work.

Agreement for Service Provider (Short-Term)
-New or current short-term employees, or all other classifications providing non-academic services beyond their normal scope of work; new short-term service providers must complete a short-term employment packet and come to HR before services begin; short-term service providers are paid one lump-sum vs. short-term hourly employees who are paid at an hourly rate and turn in timesheets.

Agreement for Service Provider (Student)
-Current student employees providing services beyond their normal scope of work; student employment requirements still apply.
