CONFIDENTIALITY

Any information you provide to Palomar College Health Services staff is protected by law as well as by Health Services policy. Medical records are confidential and entirely separate from academic records. Palomar College administrators, faculty, parents, spouses, and partners DO NOT have access to your medical records.

National privacy laws also require the safeguard of protected health information (PHI) that includes your name, address, phone number, Social Security number, other demographic information, and any information related to your health condition or diagnosis.

Medical information is released only with written consent from the client, except in the following cases:

• Certain communicable diseases and conditions
   are reported to the Public Health Department
   as required by law

• Court order
