NAME/PHOTO BADGES

Due to varying contractual agreements with the participating hospitals two badges are required:
                                                Name Badge and Photo Badge

For PHOTO Badge: Purchase a PIC card (available in the Student Services Center) and a "Easy Clip Badge Holder" available in the campus bookstore at Customer Service.

For NAME Badge(s): Complete the "Request Form" below and mail the completed form with payment directly to B & K Engraving. The badge will be sent to the Nursing Department for disbursement in class.
