How to Become a Member:     

1. Come in to the Fitness Center during our business hours.
   Complete the paperwork and purchase your membership.
   Mon. - Thurs. :      6:00 am - 8:30 p.m.
   Friday:                   6:00 am - 6:00 p.m.
   Saturday:             7:00 am - 3:00 p.m.
   Sunday:                 Closed

   OR

2. Print out and complete the forms listed below.
   **Bring, mail, or Fax the completed forms to the Fitness Center.

Membership Forms:    

1. Membership Application Form
2. Health History Questionnaire / Liability Form
3. Membership Policies and Procedures Form
4. Membership Prices and Options (FYI)
5. Payroll Deduction
   (Palomar College Classified and Faculty Staff Only).
   Deduction forms can be picked up at the Fitness Center.

Guidelines for Freezing a Membership:
Valid only for paid-in-full memberships.

• All memberships must be frozen for a minimum of one month.

• During the frozen period I understand that I may not use the Palomar College FC.

• Frozen memberships must be for entire months, beginning on the first of the month.

• "Frozen" months will be added on to my membership in the following manner:

Upon completion / submission of the Un-Freeze form all persons with "frozen" months will receive an extension of their original expiration date.

directly to Palomar College:

If paying with MC/Visa ensure that you’ve calculated your membership dues correctly and
include a completed MC/Visa Form (MC/ VISA Form) with your credit card number,
expiration date and a signature as it appears on your card.

Receipts will be kept on file.

Faxed materials without complete/ valid MC/VISA information will be held in our pending file
at the Fitness Center front desk in the pending box.

Parking:
Membership Parking Policy Form

Parking is available to all persons that are not current Palomar College students, faculty or staff. 

Parking permits are available to be picked up at the Fitness Center front desk within 72 hours.
