In order to receive the money, you must apply.

APPLICATION INFORMATION

Please take a few minutes to fill out a Participant Profile AND Confidential Profile:

1.)     The Department of Education has added a second profile.  This is new to all returning students who have received funds in previous semesters.  Each student must complete this long form when they receive Quality Improvement dollars.  ALL INFORMATION IS CONFIDENTIAL!  Once you have completed this long form, in future semesters, you will only have to fill out a short form (unless aspects of your life have changed).  Hey, more paperwork, but it is still free $$$.

2.)     Complete the Child Development Training Consortium Participant Profile AND Confidential Profile.  To access the current Profiles, click on each of the profiles above.  Every category/box must be completed on both forms or your profiles will be returned, and you wont receive the $$$.

3.)     You can also access a hard copy of the Profiles from your Child Development instructor or in MD-105, NA-3, ESC-202, and outside Dianes office MD 359.

4.)     Please complete the forms PDF forms or use in black or blue ink, print, and write carefully.  All signatures MUST be original.

5.)      ALL areas on these forms need to be filled out completely!!!  Any area left blank will be returned to you! If your forms are not complete, you will NOT be reimbursed for your classes.  ALL complete forms must be turned in by the date on the top of page 1 on the Participant Profile.

6.)     List the Child Development and General Education classes you are currently taking.  PE or Work Experience classes are NOT accepted.  All courses must be current for the semester in which you are applying and applicable to the Permit.

7.)     Have your employer fill in Section C on the Participant Profile completely.  You are also responsible for this section.  Be sure your Director/Owner fills in ALL boxes including the license number, their name and title, and signature are complete.  It is your responsibility to make sure the form if completely filled out prior to turning it in.

8.)     Staple both profiles together before you turn them in.

9.)     Return completed forms to your instructor, or to Diane Studinka in MD-359 (okay to slide under the door), or you may also mail it to:
