Please note, as stated on the Service Provider contracts, all approving signatures must be obtained before the services begin.

Service Provider Contracts:
Agreement for Service Education (Academic)
-Full time or part time Faculty, or Academic Administrators providing educational services beyond, and not directly related to, their normal scope of work (i.e. guest lecturer). If the service is directly related to their normal scope of work, then use a Notice of Hourly Employment (NOHE).

Agreement for Service Provider (Classified)
-Current Classified employees providing services beyond, and not directly related to, their normal scope of work. If the service is directly related to their normal scope of work, then the service is considered overtime. Classified employees employed as Service Providers must adjust their work schedule accordingly (see Human Resource Services for details).

Agreement for Service Provider (Student)
-Current Student employees providing services beyond their normal scope of work; student employment requirements still apply.

Agreement for Service Provider (Temporary)
-New or current Temporary employees, or all other classifications providing services beyond their normal scope of work; Temporary Service Providers are paid in lump-sums for special project type of work vs. Temporary hourly employees who are paid at an hourly rate and turn in timesheets.
